Multipurpose Senior Service Program (MSSP)
Is a care management service for seniors. The MSSP care management team is made up of a social worker and a public health nurse. MSSP care managers work with the elder and their family to find services in the home. Elders must be eligible for Medi-Cal, at risk of going to a nursing home, live in Sonoma County and be willing to accept MSSP services. Call (707)565-5970 to apply, there is usually a waiting list.
